The forms in this census package should be familiar to you - with some additions they are the forms you complete for us every year. If this is your first time completing the census package, the good news is that once you have completed this year, next year the information will be pre-populated and you will only need to make updates to it. The census has been populated with every employee we have from prior years, as well as any 2011 employees for whom we have information. Please review the census instructions below carefully - depending on the number of employees you have, you may wish to use different methods to complete it. Each form has a Warnings level to the right of the form. Levels are Red, Yellow and Green and operate in the same manner as a traffic signal. Green indicates that the form is complete and all the data has been entered in the correct format and all required data has been entered. A yellow warning indicates that some part of the form remains incomplete, but you may submit the package with this information missing. However, we may contact you later to obtain what you leave incomplete. A red warning must be resolved before the form can be submitted. All data with an asterisk must be filled in. Date fields must be complete in month, month, day, day and four digit year formats to clear. To move forward, please click on the Plan Sponsor Questionnaire. Fill out the form by checking the boxes or adding requested information. When finished click submit, which will return you to the forms page. To proceed, click on Plan Assets Summary. The Plan Assets page is designed to enter Plan Assets for all types of plans from those with recordkeepers who maintain all the assets to plans with multiple separate investments. If this is your first year with us, the first year assets are submitted electronically and you will need to add each of the assets present in the plan. If you normally check the recordkeeper box and don't provide asset values, click "Add Asset" then "add Recordkeeper" choose YOUR recordkeeper and click the box that describes your plan's assets. If all your assets are at this recordkeeper, this page is complete. Exit and return to the forms page. If you have a Defined Benefit Plan or Profit Sharing plan with a group of pooled assets, please add each of the assets in your Plan. A brokerage account is an asset – don't enter each investment in the brokerage account. Do enter single stocks or bonds that you hold outside an account. Let's move on to the third main form, Plan Deposits and Withdrawals. Skip this form if you indicated that all your assets are at a recordkeeper on the Plan Assets form. If you otherwise made Trust deposits and withdrawals during the year, begin by recording the deposits. Then enter the withdrawals. You will click on the Exit button when complete. The census is your last form. Click on the link marked "Employee Census". All the employees we have in the system will display If you have just a few employees (perhaps 10 or less) you can add 2011 compensation data by clicking on the employee name. Complete the form that comes up and click approve at the bottom. Repeat until all employees have a green or yellow status. You will click on the Exit button when complete. Add new employees by clicking on "Add Employee" from the census page. If you have more than 10 employees, click on the export/import data button. EXPORT the current census to excel to get the data AND THE FORMAT for your import. The spreadsheet will have 2 pages. One is the data page and the second page has all the explanations for what data is valid. Add compensation, deferrals, and any other column for which you have new data this year. If you have no data for a column, complete the dollar value columns with zeros. Add new employees. Save the completed file, return to the web page and look for the file you saved. Click import. If your data is correct, the file will import and any warning flags will display when you exit this screen and click on the employee census screen again. If you have a few warning flags, click on the employee with the flag and correct the problem. If you have a mass of flags, we suggest you return to your excel spreadsheet, check it for a repeated error, then upload it again. When all the employees are green, or yellow with an explanation, exit back to the view forms page. All your forms should be green. Click on Submit year end package. Verify that the answers on the next page are all "yes," and then submit the package. The next screen allows you to download a copy of what you submitted for your file.
