Form 5558 does not require your signature. It is an extension that is usually filed on your behalf and signed by Innovative Pension. You need to include a copy of the signed 5558 with your 5500 or 5500EZ filing. Innovative Pension will provide this copy for you usually by email in Acrobat format.
